`Mapper::size` & `IndexSet::size` should return size_t (or some unsigned type)

The Mapper::size method currently returns int, but I think it should return a size_t (or some other unsigned type) similar to std::vector::size.

I'm also wondering if IndexSet::size should also return a size_t instead of IndexType? It's not returning an index after all.
