[cleanup] Remove deprecated code

Carsten Gräser requested to merge cleanup/remove-deprecated-stuff into master

This removes code that was already deprecated in the 2.9 release. So far it does not remove deprecated basis implementations that are still used in non-deprecated code. This also incorporates dirichletbcassembler.hh that relies on no-longer existing AmiraMesh support in dune-grid.
