This is fallout from the recent major backport.  It is not clear
what this userdata is or what it is good for.  Apparently it is not
needed for the ReferenceElements, and neither for the general
Geometry interface.

This new patch makes sure we get compile-time improvements for
reference elements _without interface change_.

Initialize the various internal tables of ReferenceElement at program startup instead of at compile time.

This makes compiling code that uses the reference elements a lot
faster.  Hence, in particular, it fixes FS 1094.
For example, the program test-multilineargeometry now compiles about
six times faster on my laptop (with gcc 4.7).

Everything in this patch is stuff backported from Martin's branch.
So all credit's go to him.  To my git-newbie eyes the branches had
diverged too fast for actual cherry picking.  Therefore I backported
by hand.  Apologies if I screwed anything up.

This patch contains the disputed specizalization of the ReferenceElement
class with a 'void' parameter.  Since there has never (to my knowledge)
been an actual vote in favour of this change I will remove it in a
subsequent patch.
