Stop using deprecated class SubsamplingVTKSequenceWriter

Oliver Sander requested to merge feature/stop-using-subsamplingvtksequencewriter into master

Said class has been deprecated in dune-grid, because it is much more elegant to use a combination of a SubsamplingVTKWriter with a VTKSequenceWriter. This patch removes all (three) occurrences of the SubsamplingVTKSequenceWriter class in dune-pdelab.
