[doc] Fix documentation of ConvectionDiffusionModelProblem

Previously, the documentation of ConvectionDiffusionModelProblem and the ConvectionDiffusionFEM local operator differed slightly regarding the definition of b. The patch fixes this by correcting it to the implemented behaviour. Additionally, fixes template parameter documentation.
