Remove grid generator, GRAPE support and various shell utilities

This merge request removes

  • the internal grid generator (gm/gg2/, gm/gg3/),
  • the Perl modules (pm/),
  • support for GRAPE (graphics/grape/),
  • and various utilities in bin/ that no longer seem useful.

The removal of the grid generator closes issue #10 (closed).
