Remove more no longer used code

Ansgar Burchardt requested to merge ansgar/dune-uggrid:cleanup-4 into master

This merge request removes:

  • the C++ frontend for DDD,
  • several unused functions from PPIF,
  • the tools directory which provided tools to visualize UG's metagrid format and to convert different grid formats,
  • some parts of np/algebra,
  • most remaining bits of dev/*, and
  • ui/fieldio.* which was already unused.
