These are superseded by Dune::IntegralRange and Dune::StaticIntegralRange in rangeutilities.hh. The latter provide a more complete implementation can can almost be used as drop in replacement.

Since the old implementations lived in Impl:: we can safely remove them without breaking compatibility.

This implements #113 (closed).
