Clean up ISTL changes

The recent changes to ISTL weren't very clean (see also #68 (closed)). This merge request removes all occurrences of the old namespace are gone from PDELab and adds entries to the change log so that users know what they have to change. It also gets rid of an include-related warning in one of the tests.
